Mechanism of Action
Aztreonam is a monocyclic beta-lactam that binds penicillin-binding protein 3 in aerobic Gram-negative bacilli with very high affinity, blocking septal peptidoglycan synthesis and causing filamentation and lysis. Its monobactam nucleus is stable to metallo-beta-lactamases, which hydrolyse virtually all other beta-lactams. It has no meaningful activity against Gram-positive organisms or anaerobes, and its structural dissimilarity to penicillins means it is generally tolerated by penicillin-allergic patients.
