Mechanism of Action
Bedaquiline binds the oligomeric c-ring subunit of mycobacterial ATP synthase and blocks proton translocation through the F0 domain. Loss of proton motive force coupling collapses ATP generation in both replicating and non-replicating bacilli. This novel target gives it no cross-resistance with rifamycins, isoniazid or fluoroquinolones, which is why it anchors modern all-oral MDR-TB regimens.
