Mechanism of Action
Cefoperazone binds penicillin-binding proteins and blocks peptidoglycan transpeptidation, with particularly high affinity for PBP3 in Gram-negative bacilli. The piperazinedione side chain, structurally related to that of piperacillin, extends activity to Pseudomonas aeruginosa. Biliary rather than renal elimination is the dominant clearance route, which is unusual among cephalosporins and shapes its clinical positioning.
