Mechanism of Action
Diethylcarbamazine acts principally on the host arachidonic acid pathway, inhibiting cyclooxygenase and lipoxygenase-derived mediators so that microfilariae lose their protective surface and are rapidly sequestered and destroyed by the host innate immune system. It also appears to sensitise the parasite surface to granulocyte adherence. Its action is therefore substantially immune-dependent rather than directly cytotoxic, which explains its rapid microfilaricidal effect in the bloodstream.
