Triazole antifungal — topical, onychomycosis-specific
Mechanism of Action
Efinaconazole inhibits fungal lanosterol 14-alpha-demethylase, depleting ergosterol and destabilising the fungal membrane. Its distinguishing property is low keratin binding relative to other azoles, so a much larger free fraction remains available to diffuse through the nail plate to the infected nail bed. This pharmaceutical property, rather than raw potency, is what makes topical monotherapy of onychomycosis viable.
