Mechanism of Action
Entecavir is a carbocyclic 2-deoxyguanosine analogue phosphorylated by cellular kinases to entecavir triphosphate. The triphosphate competes with deoxyguanosine triphosphate and inhibits three activities of hepatitis B polymerase: priming of the reverse transcriptase, reverse transcription of the negative strand from pregenomic RNA, and synthesis of the positive DNA strand. This triple inhibition gives very high potency and a high genetic barrier to resistance in nucleoside-naive infection.
