Mechanism of Action

Ethionamide is a thioamide prodrug activated by the mycobacterial monooxygenase EthA to an S-oxide and further reactive species. The activated metabolite forms an adduct with NAD that inhibits the enoyl-ACP reductase InhA, the same target as activated isoniazid. Blocking InhA arrests mycolic acid biosynthesis, though the different activating enzyme means ethionamide often retains activity against katG-mutant isoniazid-resistant strains.
