Mechanism of Action
Fidaxomicin inhibits bacterial RNA polymerase at a site distinct from the rifamycin binding pocket, preventing open promoter complex formation and blocking transcription initiation. Its very poor gastrointestinal absorption confines exposure to the colonic lumen, and its spectrum is unusually narrow, sparing much of the commensal anaerobic flora. It is bactericidal against Clostridioides difficile.
