Mechanism of Action
Fluconazole selectively inhibits fungal cytochrome P450 14-alpha-demethylase (CYP51, ERG11), blocking the oxidative removal of the 14-alpha methyl group from lanosterol. The resulting depletion of ergosterol and accumulation of 14-alpha-methylated sterol precursors disrupts membrane fluidity and the function of membrane-bound enzymes. Its high water solubility and low protein binding give it unusually good CSF and urinary penetration for an azole.
