Mechanism of Action

Flucytosine is taken into fungal cells by cytosine permease and deaminated by fungal cytosine deaminase to 5-fluorouracil, an enzyme mammalian cells largely lack. The metabolite is converted to 5-fluorouridine triphosphate, which is misincorporated into fungal RNA and disrupts protein synthesis, and to 5-fluorodeoxyuridine monophosphate, which inhibits thymidylate synthase and blocks DNA replication. Rapid emergence of resistance means it is used in combination, classically with amphotericin B.
