Mechanism of Action

Fosfomycin is a phosphoenolpyruvate analogue that enters bacteria through the glycerophosphate and hexose phosphate transport systems and irreversibly inhibits MurA, the enolpyruvyl transferase that catalyses the first committed step of peptidoglycan synthesis. Blocking cell wall assembly at this earliest stage gives bactericidal activity and no cross-resistance with beta-lactams, aminoglycosides or quinolones. The trometamol salt provides the oral bioavailability and urinary concentrations needed for single-dose therapy.
