Mechanism of Action
Aminoglycoside antibiotic (complex of C1, C1a, C2, C2a components) that irreversibly binds the 30S ribosomal subunit (16S rRNA), causing misreading of mRNA codons and premature termination of protein synthesis. Bactericidal with concentration-dependent killing. Requires oxygen for uptake (inactive against anaerobes). Synergistic with beta-lactams for gram-positive coverage (endocarditis regimens). Nephrotoxicity and ototoxicity limit use.

Serious gram-negative infections (Pseudomonas, Klebsiella, E. coli) — often in combination, bacterial endocarditis (synergistic combination with ampicillin or vancomycin), neonatal sepsis/meningitis (with ampicillin), pelvic inflammatory disease, plague, tularemia, brucellosis (with doxycycline), topical: wound and eye infections.
