Mechanism of Action

Hydroxychloroquine shares chloroquine’s haem-polymerisation blockade in the parasite food vacuole, with an added hydroxyl group that improves its therapeutic index. In autoimmune disease it raises endosomal pH, impairing Toll-like receptor 7 and 9 signalling and antigen processing, which dampens interferon and cytokine output. This immunomodulatory action, not the antimalarial one, now accounts for the great majority of global consumption.
