Mechanism of Action
First carbapenem introduced clinically; highest in vitro PBP-binding affinity. Requires co-administration with cilastatin (renal dehydropeptidase-I inhibitor) to prevent renal inactivation and nephrotoxicity. Cilastatin does not alter antibacterial activity. Active against most aerobic gram-positive, gram-negative, and anaerobic organisms.
