Mechanism of Action

Isavuconazonium sulfate is a water-soluble prodrug rapidly cleaved by plasma butyrylcholinesterase to release the active triazole isavuconazole. Isavuconazole binds fungal CYP51 and blocks 14-alpha-demethylation of lanosterol, depleting ergosterol and destabilising the fungal membrane. The prodrug design avoids the cyclodextrin vehicle required by voriconazole and posaconazole injections, which is relevant in renal impairment.
