Mechanism of Action
Isoniazid is a prodrug activated by the mycobacterial catalase-peroxidase enzyme KatG to a reactive isonicotinoyl radical. This radical forms an adduct with NAD, which inhibits the enoyl-acyl carrier protein reductase InhA in the fatty acid synthase II pathway. Blocking InhA halts mycolic acid biosynthesis, destroying the integrity of the mycobacterial cell wall and producing potent bactericidal activity against actively dividing Mycobacterium tuberculosis.

Its dominant use is as the H component of the first-line HRZE four-drug fixed-dose combination (isoniazid, rifampicin, pyrazinamide, ethambutol) and the HR continuation-phase FDC, plus 3HP and 6H TB preventive therapy regimens.
