Mechanism of Action
Itraconazole inhibits fungal lanosterol 14-alpha-demethylase, arresting ergosterol biosynthesis and causing accumulation of toxic methylated sterols in the fungal membrane. Its extended lipophilic side chain confers far broader activity than fluconazole, including Aspergillus species and the dimorphic fungi. The same lipophilicity drives extensive tissue and keratin accumulation, which is why it is a mainstay of nail and skin mycoses.
