Mechanism of Action
Ketoconazole inhibits fungal cytochrome P450 14-alpha-demethylase, halting conversion of lanosterol to ergosterol and increasing membrane permeability. At higher concentrations it also inhibits mammalian steroidogenic P450 enzymes including CYP17A1 and CYP11B1, the basis of its off-label endocrine use and of the hepatotoxicity that curtailed oral therapy. Topically it additionally suppresses Malassezia species implicated in seborrhoeic dermatitis and dandruff.
