Mechanism of Action
First-in-class oxazolidinone; inhibits bacterial protein synthesis at an early initiation step. Binds the 23S rRNA (domain V) of the 50S ribosomal subunit, preventing formation of the 70S initiation complex (blocks fMet-tRNA binding). Unique binding site means no cross-resistance with other ribosomal inhibitors (macrolides, chloramphenicol, aminoglycosides). Bacteriostatic against staphylococci and enterococci; bactericidal against some streptococci. Weak, reversible MAO inhibitor — risk of serotonin syndrome.
