Mechanism of Action

Methenamine is excreted unchanged into urine where, at pH below approximately 5.5, it hydrolyses to formaldehyde and ammonia. The liberated formaldehyde denatures bacterial proteins and nucleic acids non-specifically, so no meaningful bacterial resistance develops even after years of continuous use. The hippurate counter-ion contributes by helping acidify the urine and sustain the hydrolysis reaction.
