Mechanism of Action
Miconazole inhibits fungal 14-alpha-demethylase and thereby ergosterol synthesis, while also inhibiting peroxidase and catalase so that intracellular hydrogen peroxide accumulates to cytotoxic levels. This dual action gives it fungicidal rather than merely fungistatic behaviour against Candida at topical concentrations. It retains useful activity against Gram-positive bacteria, which is convenient in mixed superficial infections.
