Mechanism of Action
Nystatin binds ergosterol in the fungal plasma membrane and self-assembles into transmembrane pores, allowing uncontrolled efflux of potassium and small metabolites and causing osmotic lysis. Because the target is a sterol rather than an enzyme, acquired resistance is uncommon. Its negligible absorption from the gastrointestinal tract confines activity to the gut lumen and mucosal surfaces, which is the basis for its oral use against candidiasis without systemic toxicity.
