Mechanism of Action

Oritavancin acts by three mechanisms: it binds the D-alanyl-D-alanine terminus to inhibit transglycosylation, binds the pentaglycyl bridge segment of peptidoglycan to inhibit transpeptidation, and disrupts bacterial membrane integrity through its 4-prime-chlorobiphenylmethyl side chain. The additional binding site and membrane activity preserve killing against vanA vancomycin-resistant enterococci. The result is rapid concentration-dependent bactericidal activity with a very long terminal half-life.
