Mechanism of Action
Piperaquine is a bisquinoline structurally related to chloroquine, carrying two 4-aminoquinoline units linked by a propanediamine bridge. It accumulates in the acidic parasite food vacuole and blocks haem detoxification, allowing toxic free haem to build up. Its bulky bis-quinoline architecture reduces recognition by the mutant PfCRT transporters that confer chloroquine resistance, and its very long half-life provides weeks of post-treatment prophylaxis.
