Mechanism of Action

Posaconazole inhibits lanosterol 14-alpha-demethylase with very high affinity, and its extended lipophilic tail makes additional contact with the enzyme substrate channel. This yields the broadest azole spectrum in clinical use, including reliable activity against Mucorales, which most other triazoles lack. Ergosterol depletion combined with accumulation of methylated sterol intermediates compromises membrane integrity and arrests fungal growth.
