Mechanism of Action

Pretomanid is a bicyclic nitroimidazooxazine prodrug reduced by the F420-dependent nitroreductase Ddn of Mycobacterium tuberculosis. Under aerobic conditions the activated drug inhibits mycolic acid biosynthesis and cell wall formation, while under anaerobic conditions it releases reactive nitrogen species including nitric oxide that poison cytochrome oxidase. This dual aerobic and anaerobic killing is what allows it to shorten regimens against persistent bacilli.
