Mechanism of Action
Primaquine is a prodrug whose CYP2D6-generated hydroxylated metabolites undergo redox cycling to produce hydrogen peroxide and superoxide within parasitised cells. This oxidative stress destroys the hypnozoite liver stages of Plasmodium vivax and Plasmodium ovale that other antimalarials cannot reach, and it is also potently gametocytocidal against Plasmodium falciparum. The same oxidative mechanism underlies its haemolytic liability in G6PD-deficient individuals, which shapes programmatic deployment.
