Mechanism of Action
Protionamide is the n-propyl homologue of ethionamide and follows the same activation pathway through the mycobacterial monooxygenase EthA. The activated species forms an NAD adduct that inhibits enoyl-ACP reductase InhA and halts mycolic acid biosynthesis. Its slightly greater lipophilicity is associated with improved gastrointestinal tolerability in some regimens, and it shows complete cross-resistance with ethionamide.
