Mechanism of Action

Pyrazinamide is a prodrug converted by mycobacterial pyrazinamidase (PncA) into pyrazinoic acid, which accumulates in the acidic environment of the phagolysosome. The protonated acid re-enters the bacillus, acidifies the cytoplasm and disrupts membrane energetics and transport. It also interferes with trans-translation via ribosomal protein S1, giving it unique sterilising activity against semi-dormant persister organisms that other agents do not reach.
