Mechanism of Action
Quinine is a cinchona alkaloid that accumulates in the parasite digestive vacuole and inhibits the biocrystallisation of haem into haemozoin, leaving cytotoxic free haem to damage parasite membranes. It is a blood schizonticide effective against all human Plasmodium species and retains activity where synthetic quinolines have failed. Its narrow therapeutic margin and cinchonism profile keep it a second-line and rescue option rather than a first-line agent.
