Mechanism of Action

Sertaconazole inhibits fungal 14-alpha-demethylase and blocks ergosterol synthesis in the manner of other imidazoles, but its benzothiophene ring is a tryptophan bioisostere that inserts directly into the fungal membrane and creates pores independently of enzyme inhibition. This second, fungicidal mechanism operates even against non-growing cells. It also shows intrinsic anti-inflammatory and antipruritic activity, which is a formulation selling point in inflamed tinea.
