Mechanism of Action
Sofosbuvir is a phosphoramidate ProTide prodrug of a 2-deoxy-2-fluoro-2-methyluridine monophosphate that bypasses the rate-limiting first phosphorylation step in hepatocytes. Intracellular hydrolysis by cathepsin A and HINT1 followed by kinase action generates the uridine triphosphate analogue. This triphosphate is incorporated by the hepatitis C NS5B RNA-dependent RNA polymerase and acts as a chain terminator across all major HCV genotypes.
