Mechanism of Action
Telithromycin is a ketolide that binds the 50S subunit at domain V of 23S rRNA while its alkyl-aryl carbamate arm makes an additional contact at domain II, giving a second anchoring point. This dual binding preserves activity against many erm- and mef-mediated macrolide-resistant pneumococci. The 3-keto group in place of cladinose removes the inducer of MLSb resistance.
