Mechanism of Action
Tenofovir alafenamide is a phosphonoamidate prodrug of tenofovir that is preferentially taken up intact by lymphoid cells and hepatocytes, where cathepsin A and carboxylesterase 1 release tenofovir intracellularly. This targeted delivery yields far higher intracellular tenofovir diphosphate at roughly a tenth of the plasma tenofovir exposure seen with the disoproxil prodrug. The active diphosphate is an obligate chain terminator of HIV-1 reverse transcriptase and hepatitis B viral polymerase.
