Mechanism of Action

Terbinafine inhibits squalene epoxidase, an early non-cytochrome enzyme of the fungal sterol pathway, which both starves the cell of ergosterol and causes toxic intracellular accumulation of squalene. The squalene build-up is directly fungicidal against dermatophytes, distinguishing allylamines from the fungistatic azoles. Because the target is not a cytochrome P450, terbinafine has a comparatively clean drug-interaction profile, although it does inhibit CYP2D6.
