Mechanism of Action

Voriconazole is a fluoropyrimidine-substituted derivative of fluconazole that binds fungal CYP51 with substantially higher affinity, blocking 14-alpha-demethylation of lanosterol. The structural modification extends the spectrum to moulds, giving fungicidal activity against Aspergillus species and useful coverage of Scedosporium and Fusarium. It is metabolised chiefly by CYP2C19, whose polymorphism produces wide interindividual exposure differences.
